The Family Place Utah is seeking a full-time (30-40 hours per week) Care Coordinator to join our Trauma Resiliency Project (TRP) in support of our mission, “Strengthening families and protecting children .” Care Coordinators help children, youth, and families receive needed services by gathering screening and eligibility information, connecting clients to services, providing ongoing support, and assisting with community education and engagement. Must be bilingual Spanish and English and able to effectively interact with individuals from diverse cultures.

Duties Synopsis
Care Coordinators screen children, youth, and parents for trauma, need for services, and available resources. They facilitate access to services by completion of intake and referral processes. They follow up with clients, monitor progress and needs, and provide ongoing support. They assist with community outreach and education. Care Coordinators report to the Therapy Grants Supervisor/Trauma Resiliency Project Director. Must be able to work from TFPU office locations and remotely via secure, private, HIPAA-compliant online connection as assigned.
Qualifications
1. Bachelor’s Degree in a Human Services related background.
2. Ability to pass a suitability determination and criminal background check.
3. Proficient in English: verbal, reading, writing.
4. Bilingual Spanish/English required.
5. Competent in computer use, i.e., databases, MS Word, Excel.
Skills, Knowledge and Experience
1. Experience serving trauma victims preferred.
2. Ability to handle all interactions in a trauma-informed manner.
3. Culturally sensitive, effectively engaging with individuals from diverse cultures.
4. Highly organized, take initiative.
5. Effective interpersonal and communication skills.
6. Experience working with children and adolescents preferred.
7. Experience with crisis-related services preferred.
8. Ability to maintain self -care and to work well under stress.
9. Knowledge of child and adolescent development and of positive parenting approaches.
10. Knowledge of community resources and referral process.
11. Knowledge of child abuse symptoms and their effect on victims.
